Massage Intake and Informed Consent Policy

ADULT CLIENTS AND ACCURATE INFORMATION

Jules Body Work currently accepts clients who are at least 18 years old. By submitting an intake, you attest that you are at least 18 and that the information and electronic signature you provide are your own.

Provide accurate and complete information to the best of your knowledge. Use “None” when a required question does not apply. Update the protected intake before treatment when health, medication, pregnancy status, treatment reason, requested or avoided body areas, contraindications, or other relevant circumstances change.

Massage therapy does not diagnose illness and is not a replacement for medical care. The licensed massage therapist may request medical clearance, request changes, modify a proposed plan, decline treatment, or end a session when appropriate for safety, professional boundaries, law, or scope of practice.

INFORMATION REQUESTED IN INTAKE

The protected intake may request legal name, phone number, treatment goals, requested and avoided body areas, pressure preference, allergies, medications, current conditions, recent injuries, surgeries, pregnancy status, and specified safety information. An emergency contact is optional. Medical-clearance files are accepted only after the therapist requests one through the protected workflow.

Do not place unnecessary diagnosis or health details in ordinary email, text messages, public contact forms, reviews, or Square payment notes. Use the protected intake or secure client portal.

INITIAL SIGNATURE AND POLICY ACKNOWLEDGEMENT

Before submitting intake, you must review the published Privacy, Cancellation, and Intake & Consent policy versions displayed in the form. You must acknowledge the required professional-boundary, draping, and session-ending statements and type your full legal name as an electronic signature.

The initial signature submits the intake for therapist review. It does not by itself approve treatment and does not replace the exact signed consultation described below.

EXACT CONSULTATION PLAN AND TWO SIGNATURES

Before the first approved massage session, Julie Martinez prepares and signs an exact consultation document. It records:

  • The massage services or techniques anticipated for the session.
  • Body areas included in treatment and areas to avoid.
  • Indications and contraindications relevant to the plan.
  • Required draping and session-ending statements.
  • The licensed therapist’s signed name and authenticated identity.

The client receives the same immutable consultation document in the protected portal. The client may request changes instead of signing. Treatment is approved only after the client signs the exact document using the legal name on the intake and both authenticated signatures are bound to that document.

For a later booking, the selected catalog service and its current definition must remain compatible with a structured service binding in the current co-signed consultation. A different service, a relevant catalog-definition change, a reported information change, or an in-person or off-platform change to techniques, included or avoided areas, indications, contraindications, or draping terms requires an updated protected intake and a newly co-signed consultation before treatment. No off-platform or verbal change bypasses this requirement.

DRAPING, PROFESSIONAL BOUNDARIES, AND OPTIONAL GLUTEAL-AREA WORK

The breasts of female clients remain draped. Jules Body Work does not offer or perform breast massage.

The genital area and gluteal cleavage remain draped at all times for every client. Optional professionally draped gluteal-area work requires a separate client choice and fresh consent for the particular visit. Leaving that option unchecked means no gluteal-area work and does not affect other treatment. Consent may be withdrawn at any time.

Massage therapy is professional and non-sexual. A client may ask to modify or end the massage at any time, and the therapist will honor that request. The therapist may also modify or end a session for safety, scope-of-practice, professional-boundary, or comfort reasons. The therapist will immediately end a session if a client initiates verbal or physical contact that is sexual in nature.

ONGOING VISIT CONFIRMATION

Before booking with a current approved intake, the client confirms whether health, medication, pregnancy status, treatment reason, planned service or technique, included or avoided body areas, indications, contraindications, or the draping plan has changed. A reported change requires an updated protected intake and new therapist-and-client signed consultation before payment or treatment.

The client also makes a separate gluteal-area choice for that visit and confirms current pregnancy disclosure. The visit confirmation is retained with the appointment and is not sent to the body-map recommendation feature.

REVIEW OUTCOMES AND MEDICAL CLEARANCE

The therapist may prepare and sign a consultation for client review and co-signature, request intake changes or medical clearance, or decline the requested treatment.

Health details are not placed in routine email notices. Email and portal notifications direct the client to the protected account for status and next steps. If an intake decision or intake-review deadline releases a paid appointment or package reservation, the published Cancellation policy and recorded account outcome control the related refund or package-credit restoration.

RECORDS AND RETENTION

Submitted intake versions, consultation documents and signatures, visit confirmations, treatment notes, requested medical-clearance records, and related audit events are protected treatment records assigned to a seven-year retention class. Intake drafts expire after 30 days. A client may request access using the Health Care Records instructions in the Privacy policy.

ONGOING CONSENT AND WITHDRAWAL

Consent is ongoing. You may ask questions, decline a technique or body area, withdraw optional consent, or ask to stop at any time.

Before an appointment, submit a change through the protected intake or secure client portal, or contact Jules Body Work and request access to the protected update workflow. During a session, tell the therapist directly. Do not send detailed health information through ordinary email or text.

Withdrawing consent does not erase records already required to be retained. A change or withdrawal may mean the therapist cannot safely provide some or all requested services and may require a new consultation, release, reschedule, or cancellation. Any deposit, refund, or package-credit outcome follows the published Cancellation policy and the recorded timing and reason.

POLICY VERSION AND CHANGES

The exact policy version accepted with an intake is retained with that intake. Material changes use a new immutable version. New language applies prospectively when displayed for a later intake, acknowledgement, or booking, except when applicable law requires different treatment. A material change may require renewed acknowledgement and a new signed consultation.
